見出し写真は自己免疫性胃炎（autoimmune gastritis; AIG）の体上部大弯粘膜生検です。壁細胞が残っていますが、「もうあかん、助けて」という声が聞こえてきます。直接関係ない”はずの”主細胞ももらい事故にあって消失しています。弱々しいparietal cell protrusion/pseudohypertrophy (PCP)がみられ、偽幽門腺化生・幽門腺化生が生じています。胃底腺周囲と上皮内にはリンパ球主体の細胞浸潤がみられます。今まさに進行中のAIGですが、胃底腺が残っており、腸上皮化生が生じていないので、米国の赤い教科書では早期AIGに分類されます。

The headline photo shows autoimmune gastritis (AIG), seen in a biopsy specimen from the greater curvature of the upper body.
The parietal cells are still present, but saying "Help me". The chief cells, which are not directly associated in the pathogenesis, have also been accidentally involved and lost. Weak parietal cell protrusion/pseudohypertrophy (PCP), and pseudo-pyloric and pyloric metaplasia are seen. There is a lymphocyte-dominated cellular infiltrate around/in the oxyntic glands. This is AIG in progress. Since the oxyntic gland cells are still present and intestinal metaplasia has not occurred, it is classified as early-phase AIG in the red textbook from the US.
体部粘膜とは対照的に、前庭部大弯の粘膜には炎症細胞浸潤はごくわずかで、萎縮もしていません。
In contrast to the body mucosa, the antral mucosa has very little inflammatory cell infiltration and is not atrophic.
