症例見学会

昨日は症例見学会でした。Semijailing technique を用いたステント併用コイル塞栓術を3件見て頂きました。動脈瘤の治療経験の多い慈恵医大関連の先生方でしたが、興味を持って頂けたようでホッとしました。
また、先生方のプレゼンはどれも大変勉強になりました。来て頂いた先生方とサポート頂いたストライカー社の皆さんに感謝申し上げます。

We had guest doctors from Tokyo Jukei University and related hospitals. We showed 3 cases treated with stent-assist coiling using semijail technique for cerebral aneurysms.
I’m happy that they’re interested in our technique. Also, their slide presentations were all informative. We thank all the doctors for visiting, and Stryker people to support this event.

最新の脳動脈瘤治療法

先日のVal dIsereで開催されたABC WIN seminarで発表されていた最新の脳動脈瘤治療法を紹介します。
まず一つ目はWebという新しい機器です。
これはみかんの網のようなものを動脈瘤の中に留置して、血栓化を促す機器です（図）。

ところで、こんな丸いものをどのように脳の血管を通して動脈瘤に留置するのか？簡単です。折りたたまれて細いカテーテルに入れられるので、動脈瘤の中に誘導したマイクロカテーテルから出して切り離すだけなのです。ぜひ動画をみてください。

留置法はわかりましたね。でも「コイルを詰めるのとどう違うんだ？」という質問が聞こえてきそうです。
具体的には、誘導と留置が簡単で、ステントやフローダイバーターと違って血液サラサラの薬が短期間で終了でき、しかも治療後の再発が少ないとされているのです。
「再発が少ない」「内服が必要ない」というのは安全性に関わる重要なポイントです。
この機器を使えば、これまでの方法で治療しにくかった、あるいは完治できなかった患者さんを治療できる可能性がありますが、これまでのコイル塞栓術やステント併用コイル塞栓術、そしてフローダイバーター留置術に比べて本当に優れているかどうかは今後の検証を待たなければなりません。
この機器はそれほど遠くない将来に日本で使用可能となると見込まれています。
脳動脈瘤治療の進歩もまだまだ続いていきそうです。

I'd like to introduce some new devices for cerebral aneurysm treatment which were presented at ABC WIN seminar in Val dIsere.
The first one is 'Web'.
It is a spherical dense mesh (see pictures), and promotes thrombosis when deployed in the aneurysm.

By the way, how to navigate and deployed in the cerebral aneurysm through brain vessels?
It's so simple. I can be inserted in the aneurysm and detached via a microcatheter which was navigated in the aneurysm.

OK, we understand the way of deployment. But, what was the difference between this and other embolic devices such as coilin w/wo stenting, and flow diverter?
The answer is that it is easy to deploy and we can shorten the period of antithrombotic therapy compared to stenting or flow diverter.
These are vey important in terms of safety of the treatments.
Also, we may be able to save the patients who are difficult to treat with current devices.
Its safety and efficacy should be elucidated scientifically in the near future compared to the current devices.
This, 'Web' will be introduced to our country in the near future.
Progress of cerebral aneurysm treatment will be still continued.

函館での応援講演

先日、函館で開催された講演会に行って来ました。
函館でも脳梗塞治療が増えて来ていますが、まだまだ他の地域ほどでないということで、応援を頼まれたのです。
当日は救急隊、看護師、技師の皆さんと医師の方が参加されていました。
「一人でも多くの脳梗塞患者さんを救おう！」とエールを送りました。
北海道は、冬は雪のため搬送は大変とのことで、救急隊の人が事故に合わないよう安全を確保した上で、迅速な搬送を行ってもらわなくてはなりません。
病院内で患者さんを受ける立場の人たちもその点を踏まえて頑張ってもらわないといけません。
道南の方々の熱意を感じる講演会でした。皆さん、「情熱」を持ってさらに取り組んでください。

ABC WIN at Val dIsere

今年はABC WINに参加できました。フランスのVal dIsereという山奥で開催される会ですが、ヨーロッパのトップリーダーたちが最新デバイスを用いた治療の成績を発表します。私は脳動脈瘤コイル塞栓における新しいstentを用いたSemi-jail techniqueについて発表しました。
この会で脳血管内治療の今後の方向性を知ることができて、久しぶりに刺激を受けました。ごく短時間でしたが、同行した立林先生、徳田先生とスキーも楽しめました。学会参加中にサポートしてくれた皆さん、ありがとうございました。

We attended ABC WIN this year! It was held at Val dIsere, the deep mountain in France. The clinical results of newest devices were presented. I reported our clinical results of stent assisted coiling with semi jailing technique.
We were excited to know the near future of the neuroendovascular therapy and some very new techniques.
I enjoyed skiing with Dr. Tatebayashi and Dr. Tokuda though it was a short time.
I thank all staffs for their supports during our attendance.

Rajiはナイジェリアから２ヶ月の留学で来てくれました。
すでに40代の脳神経外科医で、外科手術は相当経験があるようでした。
彼はナイジェリアに血管内治療を導入するために当科にやって来ました。戻ったら、政府関係の人たちと協力して早速取りかかるということでした。
私達の治療が参考となったのならとても嬉しいことですし、彼らがナイジェリアで治療を始めるのをサポートできたらと思っています。
Rajiは英語のネイティブスピーカーであるため、朝のカンファレンスはもちろん、現在執筆中の英語の教科書についても指導してくれました。
彼は人柄も最高です。是非母国で活躍してほしいと思います。
最終日には西宮えびす神社に行きましたよ。

Raji from Nigeria has been staying in our department for 2 months to learn endovascular therapy for cerebrovascular diseases.
He is a 40 years old neurosurgeon with sufficient surgical experiences.
The aim of his visit was to introduce neuroendovascular therapy to his country. He is going to start with the help of governmental persons.
We are happy if his visit is of help for him. We wish to support him to start endovascular therapy in his country.
Raji is an English native speaker, so he taught us in every day's morning presentation and edition of e-textbook we are working on.
He is really a nice guy. I believe that he would be successful in this field.
We went Nishinomiya Ebisu shrine at the last day.
