木下由宇先生

木下由宇先生は札幌貞心会病院から当科に1ヶ月の血管内治療の研修に来てくれました。今月は私たちの国際学会参加のため血管内治療が少なくなることを心配していましたが、急患が予想以上に多く、十分な経験が出来たとのことでした。滞在中、絵の勉強もされていて、向上心あふれるナイスガイでした。札幌に帰ってからも頑張って下さい。先生のますますの活躍を祈っています。

Dr. Yu Kinoshita from Sapporo Teishinkai hospital has been staying in our department for 1 month to learn endovascular therapy. I worried that the number of cases would be less than usual due to our attendance to the international conference, but he experienced sufficient emergency operations.
He learned drawing during his stay. He was so motivated, and nice guy.
We hope his further success, good luck!

国際脳卒中学会(International Stroke Conference）2018@LAに参加しています。
今回は発症6時間から16時間の血栓回収療法の有効性がDEFUSE3で示されました。これはこれまでのDAWN trialよりも軽症例（NIHSS6-9)が含まれており、さらに適応を広げる結果です。
また、EXTEND-IA TNKではボーラス投与可能なtenecteplaseはアルテプラーゼより血管内治療前投与においては優れていること、さらにはCOMPASS試験において血栓吸引デバイスであるPenumbraがステント型デバイスと同等の再開通効果を示し、再開通までの時間が短いことなど続々と新しい情報が発表されています。
私自身は再開通治療に関するレクチャーとRESCUE-Japan Registryの発表、当科のスタッフの発表が続き、濃厚な２日間です。
すごい発表が続き、やや疲れ気味ですが、明日までしっかりと勉強したいと思います。


We are attending International Stroke Conference 2018@LA. This time, DEFUSS 3 showed efficacy of thrombectomy from 6 - 16 h after onset. This study included the patients with mild deficit (NIHSS 6-9), so this will widen the indication of the therapy in addition to DAWN trial.
EXTEND-IA TNK showed superiority of tenecteplase, which can be administered as a bolus injection, compared to alteplase before thrombectomy.
Also, COMPASS trial showed non-inferiority of reperfusion rate of the aspiration technique (ADAPT) to stent-retriever first method. Also, time to reperfusion was significantly shorter in ADAPT technique.
Such new reports has been shown in these two days.
I made a lecture named 'Endovascular therapy for intracranial atherosclerosis' and our colleagues reported several papers.
We'll try to learn more until tomorrow without a break...

陰山博人先生と桧山永得先生の学位審査が無事終わりました。
診療の合間に本当によく頑張ってくれました。
見事に合格です。おめでとう！
二人とも４月から新たなステージに上がります。
さらなる活躍を祈っています。

Dr. Frei visited us!

本日、DenverのFrei先生が当院に来てくれました。
急性期脳梗塞のカテーテル治療に関して、極めて迅速に高い再開通率で治療されており、その結果に驚きました。
年間275例の患者さんに血栓回収療法を行って、治療時間は平均的17分、再開通率が96%で、予後良好患者さんが63%だったそうなのです！
私たちよりもはるかに良い治療成績ですので、色々な技術を教えていただきました。
今後の治療に役立てたいと思います。
この度のFrei先生の訪問に関し、関係者の皆様に心から御礼申し上げます。

Dr. Frei visited us, and made a wonderful lecture regarding acute stroke intervention.
His clinical results were excellent. Door to reperfusion was 17 min (median), and reperfusion rate (TICI 2c/3) was 96%, and good outcome @ 90 days was 63% !!! This was much better than our results.
We learned a lot of tips of the treatments from him today, so we'd like to apply them from now on.
