先日のVal dIsereで開催されたABC WIN seminarで発表されていた最新の脳動脈瘤治療法を紹介します。
まず一つ目はWebという新しい機器です。
これはみかんの網のようなものを動脈瘤の中に留置して、血栓化を促す機器です（図）。
ところで、こんな丸いものをどのように脳の血管を通して動脈瘤に留置するのか？簡単です。折りたたまれて細いカテーテルに入れられるので、動脈瘤の中に誘導したマイクロカテーテルから出して切り離すだけなのです。ぜひ動画をみてください。

留置法はわかりましたね。でも「コイルを詰めるのとどう違うんだ？」という質問が聞こえてきそうです。
具体的には、誘導と留置が簡単で、ステントやフローダイバーターと違って血液サラサラの薬が短期間で終了でき、しかも治療後の再発が少ないとされているのです。
「再発が少ない」「内服が必要ない」というのは安全性に関わる重要なポイントです。
この機器を使えば、これまでの方法で治療しにくかった、あるいは完治できなかった患者さんを治療できる可能性がありますが、これまでのコイル塞栓術やステント併用コイル塞栓術、そしてフローダイバーター留置術に比べて本当に優れているかどうかは今後の検証を待たなければなりません。
この機器はそれほど遠くない将来に日本で使用可能となると見込まれています。
脳動脈瘤治療の進歩もまだまだ続いていきそうです。
I'd like to introduce some new devices for cerebral aneurysm treatment which were presented at ABC WIN seminar in Val dIsere.
The first one is 'Web'.
It is a spherical dense mesh (see pictures), and promotes thrombosis when deployed in the aneurysm.
By the way, how to navigate and deployed in the cerebral aneurysm through brain vessels?
It's so simple. I can be inserted in the aneurysm and detached via a microcatheter which was navigated in the aneurysm.

OK, we understand the way of deployment. But, what was the difference between this and other embolic devices such as coilin w/wo stenting, and flow diverter?
The answer is that it is easy to deploy and we can shorten the period of antithrombotic therapy compared to stenting or flow diverter.
These are vey important in terms of safety of the treatments.
Also, we may be able to save the patients who are difficult to treat with current devices.
Its safety and efficacy should be elucidated scientifically in the near future compared to the current devices.
This, 'Web' will be introduced to our country in the near future.
